Download, print, sign, and mail your NMB authorization card.

Signing authorizes the Guild to petition the National Mediation Board to hold a representation election. It is not the election and it is not a vote — it is the act of embracing the change, and it takes minutes.

Sign and send — step by step

Six steps, a few minutes, one envelope.

    Download the authorization card

    Open the approved card below and save it to your phone or computer. Use only the current approved card — do not alter its language.

  2. 2

    Print it

    Print the card on a clean sheet of paper. A home printer or a library printer both work.

  3. 3

    Fill in your details

    Print your full name, your base or domicile, and your email in the spaces provided, and write the date.

  4. 4

    Sign it by hand in ink

    Sign squarely on the line with a pen, in your own handwriting. Only a wet-ink signature counts — digital, typed, or electronic signatures cannot be accepted.

  5. 5

    Seal it in an envelope

    Place the signed card in a plain envelope and seal it. Your employer will never know you signed — your right to sign is protected by law.

  6. 6

    Mail it to the Guild

    Mail the sealed envelope to the address below. You can also hand it directly to a Guild member or your base Guild Advocate instead of mailing.
